Program Goals

  • Build an aligned network of SNFs through a Preferred Network leveraging shared skills of diverse physician practices, community and tertiary hospitals and SNFs.
  • Partner with facilities who meet the standard for inclusion in the Preferred Network based on beneficiary needs.
  • Promote and support high quality, patient-centric care across transitions.

MSSP SNF 3-Day Rule Waiver Program

Program Goals

  • Improve quality of care and patient satisfaction by providing patients with the appropriate level of care based on their illness acuity or functional level.
  • Allow clinically appropriate patients to be admitted to SNF level care without a 3-day qualifying inpatients stay in an acute care setting.
  • Admit patients directly to SNF level of care from the emergency department, observation status, home or physician’s office when clinically appropriate

Preferred Home Care Network
Program Goals

  • Build an aligned network of Home Care agencies through a Preferred Network to improve transitions of care and ensure a high level of care
  • BIDCO partners with agencies that achieve and maintain a CMS Quality Star Rating of 3 or higher and are highly utilized by the BIDCO community.

PatientPing
PatientPing allows real-time admission and discharge notifications to a patient’s provider(s). It also allows admission sites to identify a patient’s ACO and gain access to the patient’s care team.

BIDCO’s Preferred SNF and Preferred Home Care Networks are required to use PatientPing. For more information on PatientPing or request access, please contact Julianne Hunn at jhunn@bidmc.harvard.edu
